Did you measure the OAL of the shells?  IDK, but looks to me like those shells were fired out of battery somehow.  Probably not the case, but if the shells are too long for your chamber and the gun does not have some mechanical means to prevent firing out of battery, the area just in front of rim would be unsupported by chamber?????  I sure wouldn't fire it again until getting to the bottom of the cause.
