Barrel accuracy issues

Ok I would talk to your Smith about this. So he headspaced it to your action then I take it? Also what bullets are you shooting and what powders, primers, cases and were they new specially when getting fliers or where they new during load development? Double check your scope or pull one off you know is good, bedding? I would talk to the smith and see what he says might be a reason the last guy pulled that barrel off.
 
55gr and 60gr bullets (Sierra and Nosler), H380 and Varget, with REM 9 1/2 and FED 210 primers.

Confirmed scope is solid, and action is bedded properly.
 
I don’t know what to tell you maybe there’s a reason it was a used barrel. Hate saying that but I think every barrel company has had a bad one before.
 
Are you using sand bags or a bipod?? A bipod could cause a bedding shift and cause flyers, especially with the cheaper stocks that don't have the aluminum bedding block. Or a wood stock if ya don't have a light hold on it every time. I'm guessing it's some sort of bedding problem. If the barrel was junk 4 wouldn't touch, or even be close.
