Lee Collet Die problem

Last night i decided to load some NEW American Eagle Brass (223). My normal procedure is to run them through my Lee Collet Neck Sizer which I've done thousands of times just to make sure the neck is good and round.

I have my dies mounted permanently on a a Redding T-7 Press.

After doing about 20 rounds the handle of the press started sticking at the bottom of the stroke with the brass fully pushed into the die . At this point the brass was being stretched significantly and to the point that the shoulder was pulled out to one big radius. At this point I took the die apart and every thing seemed fine. Ran one more round in and the die pulled the neck off the case. The neck is now stuck on the mandrill.

This die has been working fine and I actually used it earlier this week on 40 rounds out of the same bag of brass.

Has anybody seen this before?
 
Yes I’ve had them stick before, I took them apart and polished the tapered parts inside that squeeze the collet together, I also lube them periodically with a light grease.
